泌尿腫瘤學
推薦老師:泌尿科蒲永孝老師

        生殖泌尿道的腫瘤,包括腎上腺、腎臟、輸尿管、膀胱、前列腺、尿道、陰莖及睪丸等,但是女性生殖道器官,例如卵巢、輸尿管、子宮及陰道等,則屬婦科學範圍。

        國人較常見的泌尿腫瘤包括:前列腺癌、膀胱癌及腎臟癌。一般來說,相較於其它癌症,如肺癌、肝癌等,泌尿腫瘤治療後的存活率尚稱不錯,一半以上的病人可以治癒。希望藉此次書展,引起大家對於「泌尿腫瘤」的相關議題感到興趣。

The relationship between prostate cancer and androgen status was initially appreciated in the late 1800’s, and then awareness reawakened in the 1940’s by the studies of Charles Huggins and colleagues, for which the Nobel prize was awarded in 1966.   The purpose of this book is to provide an up to date review of the molecular and cellular aspects of androgen action in prostate cancer. It includes an overview of the role of androgens in the development of the normal prostate and progression of prostate cancer. Our understanding of androgen metabolism and efforts to target androgen metabolism in the prevention and treatment of prostate cancer is presented. Androgen receptor structure/function relationships and the role of co-regulators are defined. Ligand-independent activation of the androgen receptor and the role of the androgen receptor during progression to the castration recurrent phenotype are discussed. Androgen-regulated genes are elucidated and their roles during prostate cancer progression are defined. Finally, the androgen receptor is presented as a viable target for novel therapies. Together, these reviews should give the reader a comprehensive conceptual framework of androgen action in prostate cancer.

It is a comprehensive guide to current standards of care and emerging technologies, in four sections covering staging of UC, treatment for localized disease, treatment for systemic disease, and efforts in moving the field forward through translational research. Careful attention is paid to literature review and level of evidence throughout. Purpose:The purpose is to generate innovations in the diagnosis and management of bladder cancer as well as educate urologists who may not have a large practice in the management of urothelial carcinoma. As the authors note, "our practice patterns in 2008 are surprisingly similar to those in 1988." This book seeks to propel modernization in the field. Audience:Although physicians who treat bladder cancer, primarily practicing urologists, are the targeted audience, medical oncologists should also strongly consider adding this to their libraries. Residents would benefit from this book, particularly the first three sections, as an adjunct to more general urology textbooks. The authors are well versed in the field of bladder cancer treatment and basic research. Features:This book takes the reader through a comprehensive discussion of the management of bladder cancer once a diagnosis has been made. It starts with a discourse on proper staging, noting that very often the disease is not properly staged. Unfortunately, many of the radiographic techniques mentioned in the book are not widely available to the community practitioner and may have been better presented towards the end of the book with the other cutting-edge treatment modalities. The next two sections focus on treatment of clinically localized and distant or locally advanced disease. Part four focuses on translational research as the future for successful management of this disease. For clinician urologists, this last section is very helpful to understand what is taking place in academic and research centers across the globe. Also particularly useful are the chapters devoted to surgical techniques to improve quality of life outcomes after cystectomy and urinary tract reconstruction. Assessment:Overall, this presentation on bladder cancer is a valuable addition to the library of any practicing urologist. The chapters are relatively short and easy to navigate to use as a quick reference or to read cover to cover. The authors strive, successfully, to provide the latest data based on high levels of evidence and to admit where the data needs to be more robust to support paradigm shifts in the diagnosis, management, and eradication of bladder cancer.

Advances in diagnosis have led to many tumours being identified at an asymptomatic stage. There have also been parallel advances in treatment, such as nephron sparing procedures and radiologically guided ablative procedures. However, these changes have brought with them dilemmas such as our ability to evaluate the nature and aggressiveness of early stage tumours and how best to identify those patients who will benefit most from early treatment. Therefore it is increasingly important that those faced with difficult clinical questions make the right decisions. Since the first volume was published in 2000, many debates persist but new controversies have also emerged, reflected in the chapters of this new volume. New authors have been recruited to give their insight into newer areas of this controversial topic.   Challenges in prostate cancer provides a series of state-of-the-art review articles, each of which addresses and answers a contentious question. It includes chapters on medical and surgical management of all stages of prostate cancer, clinical investigation, epidemiology, clinical trials and basic scientific research and covers issues such as the genetic basis of cancer, clinical economics and influence.   The authors have used their expertise to distil the available evidence into practical advice to be used by busy urologists in their day-to-day practice. And as well as being an overview of current practice many chapters attempt to give insights into the future direction of prostate cancer research and clinical management.

The neoplasms covered include clear cell carcinomas, papillary cancers, nonepithelial tumors, and other mass lesions that resemble tumors. The management of patients with renal cancer having localized or advanced disease are discussed. Surgical approaches for primary and metastatic tumors, symptom palliation, and systemic therapy for metastatic disease including immunotherapy and targeted approaches are discussed in detail.

[et al.] Lippincott Williams & Wilkins 2006 WJ160 C738 2006 Basic bookpic Reviewer:Martin I Resnick, M.D.(University Hospitals of Cleveland) Description:This is the third edition of a book that updates the reader on new developments in oncologic diseases of the genitourinary tract. The multieditor, multiauthored book includes recognized contributors in the fields of urology, radiation oncology, and medical oncology. Two new editors have contributed to this edition but the intent of the first edition has been followed: "...to apply different areas of scientific knowledge to the goal of eradicating genitourinary cancers...." Purpose:The purpose is to provide a comprehensive review of genitourinary malignancies with discussions ranging from screening and early detection to the management of localized and advanced disease. Surgical developments are emphasized as are those in radiation and medical oncology. Because of the many developments over the past decade, the providing an update is worthwhile and of value. The editors and the individual authors have met their objectives. Audience:The book should be of interest to all healthcare providers with an interest in genitourinary oncology. This includes those in primary care but also in specialties such as urology and medical and surgical oncology. Those in training and those having completed their training should be interested in the content of the book. Additionally, other healthcare providers such as nurses, physicians assistants and those caring for patients with these diseases should also find components of this book both interesting and of value. The editors are all highly respected in their particular fields enjoying both national and international reputations. Many of the authors of the individual chapters are also highly recognized individuals. Features:Malignancies of the genitourinary organs are reviewed and topics such as disease etiology, screening and early detection studies, patient evaluation leading to diagnosis and staging, and management of localized and advanced disease are included. Methods of treatment such as surgery and radiation therapy are discussed and the pros and cons of conflicting approaches are addressed appropriately. Having editors of varying specialties helps to ensure these latter points. Assessment:This new edition is a welcome addition to the literature on this common and important complex of diseases. The varied specialties associated with the management of these patients are well represented in an appropriate manner and the material will be of value to those caring for these patients. An update was needed and the editors and authors certainly have met their goals.
7 Contemporary issues in prostate cancer : a nursing perspective / [edited by] Jeanne Held-Warmkessel Jones and Bartlett Publishers 2006 WY164 C7605 2006 Advanced bookpic Reviewer:Ayansa Zambodla, BA, RN, MA(Mount Sinai Medical Center) Description:This is a good overview of prostate cancer from epidemiology-etiology to symptom management of advanced disease. A final chapter on psychosocial and support resources is well researched. A unique element in the book is the chapter dedicated to a patient's perspective on diagnosis and bereavement. Purpose:The stated purpose is to address nursing care and management of patients with prostate cancer and to supply the reader with current information for use during patient education sessions. As communities become more aware of prostate cancer and incidence rates for men continue to rise, it is important that healthcare providers have the information available to use to better provide accurate patient education. This information is presented in this book in a writing style that is easy to read. The editor's objectives are met. Audience:The editor states that she is writing for nurses. Still, the book could be used by any healthcare professional or student seeking information on prostate cancer. The editor is a nurse writing from a nursing perspective so she is an authority on the subject matter. Features:She discusses prostate cancer from its epidemiology to risk factors, prevention, diagnosis, treatment, and decision making to advanced prostate cancer symptom management. Where relevant the editor discusses nursing care and patient education strategies. By adding a chapter on a patient's perspective she is able to focus on the fact that it is the "person" with the illness being treated. The chapter on screening and early detection includes discussion of barriers to screening but there are no suggestions for overcoming these barriers. Assessment:The book is informative and easy to read. With this resource book for educating healthcare providers on prostate cancer, the editor does a good job presenting the facts on the disease and discussing nursing care and psychosocial and support resources. She states that there is no other textbook that specifically addresses nursing care and management of patients with prostate cancer from a nursing perspective. Therefore there is no other book written from the same perspective for comparison.

Proving the reader with the latest treatment standards of prostate cancer, this book will be of great value to urologists and radiation oncologists as well as to all others involved in the care of patients with prostate cancer.

Over 80 international authors from respected institutions and varied disciplines, including urology, radiology, surgery, science, and bioengineering, contribute the 42 well written chapters. Purpose:The book covers the enormous advances in cross-sectional imaging in CT, PET, CT-PET fusion, MRI, and nuclear imaging which continue to rapidly evolve. In addition to serving as a comprehensive state-of-the-art book for clinicians working in a multidisciplinary urologic oncology setting, it gives academic clinicians and scientists a glimpse of where the field is headed. Audience:The target audience is healthcare professionals who specialize in the assessment and treatment of patients with urological malignancies, including urologists, radiologists, oncologists, and specialized ancillary staff such as technologists and nurses. The book may be useful to community and academic physicians seeking an in-depth understanding of urologic imaging in oncology. Features:The eight sections cover adrenal, renal, upper and lower tract urothelial, prostate, testes, and penile carcinomas. The last chapter is devoted to future directions in molecular, robotic, ultrasound, contrast media, and virtual imaging advances. In each section, the chapters provide clinical background, pathophysiology, and an individual chapter dedicated to specific imaging modalities. Each section devotes a final "considerations" chapter that discusses the impact of imaging technology on the practice of urology. The adrenal imaging "considerations" chapter outlines a very useful imaging algorithm for the characterization of incidental adrenal masses and it specifies when to use CT, MRI, and PET scans. The one limitation is that imaging algorithms are underutilized. For example, the evaluation and Bosniak classification of renal cysts are very familiar to urologists, yet concrete guidelines are lacking in this area. Assessment:This book provides a comprehensive, state-of-the-art discussion of all imaging modalities relevant to specific urologic malignancies. It is an excellent resource for urologists, urologic oncologists, and radiologists, and it may be especially useful for clinicians who work in a multidisciplinary urologic oncology setting.

The former is four times more prevalent than the latter. Superficial bladder cancers do not invade the muscle wall while invasive tumors do. Invasive bladder tumors have a propensity to metastasize and spread to other areas of the body and are more likely to be fatal than superficial tumors.   Invasive Bladder Cancer presents state-of-the-art diagnoses and treatments available for bladder cancer that has metastasised into the body in a full color book with over 40 tables and 50 illustrations. Prostate cancer is the most common with about 234,000 cases each year in the United States. Men 40 and over are at risk for getting this disease. The older the male, the more likely he is of developing prostate cancer. Someone aged 69 to 79 has a 1 in 7 chance of getting this disease. Prostate cancer can be inherited but can also be caused by the environment such as being exposed to cadmium (a toxic metal found in batteries, plastics and other industrial products). Pesticides have been found as a cause also. In addition, one's diet is associated with causing cancer. Eating foods high in fat, not eating enough fish, and a diet of few soy products can increase chances of prostate cancer. The best way to detect prostate cancer is through a digital rectal exam.    Furthermore, doing a PSA (prostate specific antigen) test will help determine if there is cancer. Prostate cancer can be treated through the removal of the prostate, beam radiation, or planting radiation seeds into the body. This well-written, educational book has a personal touch to it because the author has included stories about friends, acquaintances, and his uncle who had prostate cancer. This book would be a great resource for health or biology majors, pre-med students, and anyone whose family has a history of prostate cancer. This book is part of the "Deadly Diseases and Epidemics" series.

Audience:This is a must read for those who are about to enter the field of translational research in prostate cancer, but it also serves as an excellent reference for established investigators. Features:The 26 chapters are divided into three parts: basic biology and imaging, genetics, and therapy. In part I, 13 chapters cover topics ranging from prostate cancer stem cells to bone metastasis and from androgen receptor functions to novel molecular signaling events. Of particular interest, the editors have selected practical technologies such as xenograft models, tissue microarrays, nanotechnologies, and modern noninvasive imaging techniques. As pointed out by the editors, in the post-human-genome era, much progress has been made in understanding the genetics of human prostate cancer. The five chapters in part II offer readers the latest information in genetic and epigenetic advances in this area. Part III devotes eight chapters to the most innovative ideas for treating patients with advanced prostate cancer. Although effective therapeutic strategies are not yet available for advanced prostate cancer, the Lance Armstrong effect described by Don Coffey in the foreword fits perfectly for this book. Assessment:It will be interesting to look back, say in 10 years, to see whether these chapters have paved the way for the development of new and efficacious therapeutic procedures for prostate cancer.

Interwoven into these discussions are the various different healthcare systems in different parts of the world which play a significant role in how they fund for PSA screening. Since no tumor marker is perfect, new biomarkers are identified and discussed, and suggest that the future of prostate cancer screening may be based on a panel of markers and nomograms of various risk factors. Audience:This book is most suitable for statisticians, scientists, or clinicians who wish to draw their own conclusions regarding PSA screening and related topics. Chapters reach various different conclusions, so it is up to readers to make their own decisions about the value of PSA screening, especially as clinicians. Features:Chapters are well written, complete, thoroughly detailed, and cover a wide range of studies from experts all over the world. The first few chapters deal with the results of PSA screening from the U.S., the U.K., Canada, and European studies. A few excellent chapters demonstrate the use of nomograms as prostate cancer predictive tools, and PSA velocity studies which correlate with prostate cancer aggressiveness. The chapters on artificial neural networks for optimizing prostate cancer risk prediction, transcriptional profiling, and novel biomarkers were fascinating and demonstrate that the future is here. The chapters on integration of risk assessment in prostate cancer screening and family history are relevant for any urologist practicing. Graphs, charts, and illustrations are used well throughout the book and summarize many of the discussions in a simple, organized manner. Assessment:This book is a superb resource for anyone interested in PSA screening, novel prostate cancer biomarkers, and risk assessment programs. It is also an excellent reference for anyone interested in comparing the various studies and controversies from different healthcare systems around the world.

It is divided into 15 chapters, six of which contain a total of 45 subsections reviewing the data and collective expertise of the editors and authors. Purpose:The surgical and medical management of kidney cancer has undergone a significant evolution in the last decade. In this book, the authors review both the data and the details underlying these changes. While the primary purpose appears to be to highlight innovative techniques and technologies in the diagnosis and surgical management of the disease, a secondary but relevant purpose is to review hereditary factors and systemic therapies ranging from immunotherapy and vaccines to antiangiogenics and trends in medical management. Audience:The book is written for practitioners at many levels of their career including residents and fellows as well as junior and senior staff. Although its main purpose is to review new techniques and technologies in the treatment of localized and advanced disease, there is something for everyone, including urologists, radiologists, and radiation and medical oncologists. Features:Using a logical and orderly progression, the book begins with chapters addressing epidemiology, natural history, pathology, staging, and prognostic markers, then moves on to the details of diagnosis and surgical and medical treatment. More than half of the 45 subsections cover specific surgical and/or technical topics, many of which are less frequently given chapter status in similar texts (i.e. the one on coumadin and kidney cancer). Although the table of contents and the use of sections fail to convey the book's overall structure (major headings are not emphasized or separated, and/or are missing (no topic grouping chapters 10a through 10h.ii)), the internal editorial aspects and features (figures/tables/graphs/references) are well organized and presented. The radiographs and figures illustrate and complement the text nicely. As with any major book in a rapidly evolving field, some of the data, particularly regarding the medical management of advanced disease, are several steps behind the most current thinking. This is perhaps most notably reflected in the medical management sections where there is an absence of key chapters dedicated to tyrosine kinase inhibition, the role of mTOR inhibitors and emerging level I evidence on targeted therapies in RCC. Assessment:The strength of this book is based on the collective international experience of its editors and authors. It probes more deeply into diverse and less often detailed aspects of kidney cancer than similar books, ranging from obesity and management of disease in the elderly to HIFU, photodynamic therapy,PET scanning, bench surgery, and margin assessments. The book offers readily accessible pearls of wisdom to both the casual and detailed reader.

Each chapter reads as an independent, concise component of the whole, highlighting the most salient discoveries regarding tumor markers and targets as well as their clinical correlates. The first edition was published in 2006. Purpose:The convergence of two separate but interactive forces have irrevocably altered medical discovery. The first is information technology, which not only makes medical communication more efficient, but also has powered molecular screening techniques exponentially. The second is the successful clinical application of subcellular targeted therapies in cancer medicine. Nowhere have these forces come together more synergistically than in pathway discovery and approved treatment modalities for kidney cancer. While books often represent data which are a step or two behind contemporary thinking, the editors manage to keep this second edition apace with current knowledge in a rapidly evolving field. The focus of their book is decidedly translational. Audience:The primary audience includes individuals on the cusp of translational and clinical discovery in kidney cancer. For many practicing urologists, the majority of chapters represent interesting discussions and pertinent background to contextualize their care. Alternatively, for most practicing medical oncologists, the book provides essential data upon which their daily clinical decisions are based when caring for patients afflicted with kidney cancer. Features:The book takes readers through the identification of the molecular genetics of inherited forms of renal cancer and through each pertinent target in sporadic kidney cancer currently in clinical application and many in clinical trials. It goes beyond the most common targets and agents to include many emerging pathways such as the PI3K/Akt/mTOR connection, proteasomal NF-kappa B signaling, the role of hepatocyte growth factor, and Smac/DIABLO as a proapoptotic molecular target in renal cell cancer. Moreover, it includes a discussion of novel targets such as integrin fN5fO1 and EphA2 that are often omitted from more standard books. The emphasis of the chapters is moving these discoveries into the clinics and contextualizing the pathways for practicing physicians and scientists. This emphasis on targeted therapies and pathways is skillfully completed while maintaining a contemporary focus on the known immunological pathways and therapies which remain the only, albeit uncommon, cures for metastatic kidney cancer. Here the role of chemokines, interferons and interleukins, TNF and endogenous immunosuppressors are discussed in the context of their implications in this targeted therapy era. Finally, the editors bring it all back to the clinics with a complete discussion of potential molecular markers and anti-angiogenic adjuvant therapy strategies. Assessment:The strength of this book lies in its authoritative contributors, its relevance for today, and its sharp insights into a rapidly evolving field. There is little doubt this book will remain current for several years and serve as a summary reference of where the field is and where it is going.

Features:The author provides a comprehensive statement of the problem and pathophysiology of sexual function specifically related to prostate cancer. The book reviews the efforts made to preserve sexual health prior to iatrogenic causes of erectile dysfunction with chapters dedicated to potency-sparing radiation therapy, nerve-sparing radical prostatectomy, and robotically assisted laparoscopic prostatectomy. It presents contemporary approaches to penile rehabilitation using oral and local therapies and includes a chapter considering testosterone replacement therapy in the hypogonadal male following prostate cancer treatment. Assessment:The prevalent clinical concern about sexual dysfunction in prostate cancer patients highlights the importance of this topic. This is the first book to address this need and does so with a breadth and depth that provide clinicians with the confidence to reassure and treat their patients in a relevant and contemporary manner.

This new book provides up-to-date information that should be of interest to all healthcare providers caring for patients with this disorder. Purpose:The editors desired to provide a current and relevant book addressing urothelial tumors of the bladder. The book is one of few totally dedicated to this problem and is the most current. The objectives of the editors have been met. Audience:This should be of interest to all caring for patients with bladder malignancies. Established physicians in urology, radiation oncology, and medical oncology all will find interesting components of this book. Those in training in these specialties will also find this book useful. Other physicians such as those in primary care will also find this to be an excellent reference that will be of value when counseling patients with bladder cancer. The editors are well known in the field as are many of the authors of the chapters. Features:The book covers all aspects of bladder cancer including epidemiology, pathology and molecular biology, diagnostic procedures, and the multiple single and combined treatment modalities available to these patients. The chapters are well written and the tables and figures are most appropriate. The figures are well done and are clearly reproduced. Assessment:This is a welcome addition to the literature and will likely be found in the libraries of urologists, radiation oncologists, and medical oncologists caring for patients with this common disease. It provides current and relevant material that is presented clearly both in text, figures and tables.

The chapters are grouped into three distinct sections covering general considerations affecting treatment options for patients and management of localized and advanced disease. Purpose:The authors present a comprehensive review in the care of patients diagnosed with prostate cancer. They include information to aid in patient counseling as well as a thorough and virtually complete discussion of the management of patients with localized prostate cancer and those with advanced disease. Audience:The book is geared toward individuals with an understanding of the fundamentals in treatment of patients with prostate cancer and who also likely have some experience in the field. Features:The numerous illustrations and images aid in the discussion and comprehension of the material. The illustrations in chapter 10 are impressive: anatomic radical retropubic prostatectomy. Without training in a surgical specialty, I was able to gain a basic understanding of the surgical approach. Chapter 3 provides information that would benefit attending physicians and residents alike when patients present with questions regarding lifestyle modifications. Several of the chapters seemed somewhat redundant and could have been consolidated into one chapter, such as the discussion of the optimization of hormonal therapy and the attendant controversies. The three separate chapters that discuss robotic prostatectomy also could be condensed into one. Assessment:The size of the book allows for easy transport and the presentation is such that the book could be read in its entirety or used as a reference.

Controversial topics are addressed and presented appropriately. Purpose:The purpose is to update readers on new developments (clincal and research) in malignancies of the genitourinary tract. An update such as this is important. Audience:The book is written primarily for experienced physicians caring for patients with genitourinary malignancies. Urologists, medical oncologists, and radiation oncologists will all find the material to be of interest. Residents and registrars in these particular specialties will also find some of the topics of interest depending on their area of interest. The editor is a medical oncologist; the authors represent varied specialties. Features:The major malignancies of the genitourinary tract are covered, including the prostate, bladder, kidney, testes, and penis. Though many important areas are addressed, the role of radiation therapy in managing these malignancies is presented to only a limited degree. For instance, the role of radiation therapy in prostate cancer or in bladder cancer is not discussed. Approximately half of the book is devoted to prostate cancer and the remaining portion to the other malignancies. Research areas are also covered in all of the malignancies discussed. Assessment:This book brings readers up-to-date in various clinical and research areas in malignancies of the prostate, bladder, kidney, testes and penis. The topics addressed are important and appropriate for those interested in these various areas. Those with a particular area of clinical or research activity will find parts of this book to be of value.

Purpose:The purpose is to increase understanding of the basic science related to clinical situations in a multidisciplinary, integrated approach to urological malignancies in order to assist residents and specialists in urologic oncology with patient care. These are certainly worthy objectives and the editors have succeeded in creating a timely and worthy addition to urologic books by amassing an international panel of authors to provide worldwide consensus perspectives on urological cancers. Audience:Primarily aimed at urology residents, specialists in urology, oncology, and urologic oncology, the book will likely prove most beneficial to residents, fellows, and junior faculty in urologic oncology to help them understand principles of therapy and an integrated approach to management of urological malignancies. Features:The book is divided into three parts, covering basic science, clinical aspects and investigations, and systemic genitourinary oncology sections. The basic science section provides a concise yet fairly comprehensive review of the scientific principles underlying urological cancers and allows for a broader understanding of the information that follows. The clinical aspects and investigations section reviews radiology, anesthesia, laparoscopy, clinical trials, chemotherapy, radiation therapy, and even the role of diet in genitourinary cancer. Highlights of this section include excellent representative images encompassing the breadth of urologic malignancies, as well as organized and concise tables that can be quickly used as a reference by clinicians, including an overview of indications for biopsy in urologic cancers. Finally, the systemic genitourinary oncology section comprehensively reviews renal cell carcinoma, urothelial cancer, testis cancer, prostate cancer, penile and urethral cancer, and even nonurological cancers affecting the urinary tract. This section is well organized and an excellent reference for clinicians. In particular, use of contemporary articles regarding PSA velocity provides controversial and timely information for clinicians counseling patients with prostate cancer. Color images, including excellent histology slides in the chapter on bladder cancer, further enhance the information but are generally underused throughout the book. Assessment:This book is engaging, well organized, and concise without sacrificing completeness. It is a worthy companion to the more comprehensive and commonly used urologic textbooks, including Campbell-Walsh Urology, 9th edition, Wein et al. (Elsevier, 2007), and Comprehensive Textbook of Genitourinary Oncology, 3rd edition, Vogelzang et al. (Lippincott Williams & Wilkins, 2006). This book will be an outstanding resource for oncologists in training, particularly those in urologic oncology fellowships.
