腦與腸的對談

        腦腸軸(Brain-gutaxis)一詞起源於臨床症狀的觀察,即精神壓力會導致腸道的功能性失調。廣義的解釋是腦和腸之間藉由神經傳導與免疫、內分泌機制進行雙向溝通。臨床上常見腦腸軸失調的疾病包括「腸躁症」、「功能性消化不良症候群」、「胃食道逆流疾病」、「週期性嘔吐綜合症」等。其中「腸躁症」是現代社會壓力下常見的疾病,其發病率相當高,約占腸胃科門診人數的10-40%。「腸躁症」病徵是腹部的疼痛或絞痛,合併有腸道功能的改變,包括脹氣、腹瀉或便秘。其原因不明,且無生物標的,利用內視鏡也觀察不到腸道病變,因此過去「腸躁症」被視作精神疾病。最新研究得知「腸躁症」病人對精神壓力的抗性較低,較易出現恐慌和憂鬱症狀,且腸道有低程度發炎、共生菌增生和上皮屏障失常的現象。
